# I put the GLP-1 medicines in a different conversation

[Peptides Blog](/) / Semaglutide and tirzepatide

October 27, 2026

I don’t want “peptide” to do all the thinking for us. Semaglutide and tirzepatide have FDA-approved products and trials in people. That puts them on different ground from the experimental compounds elsewhere on this blog. It doesn’t give every peptide a share of their evidence.

Semaglutide copies the action of GLP-1, a gut hormone involved in appetite and blood sugar. Tirzepatide acts on the receiving points for GLP-1 and another gut hormone, GIP. Two targets describe how it works. They don’t settle which medicine fits a particular person.

## A medicine can change an ordinary meal

![At a weekday stove, an adult spoons a small portion of pasta onto a plate.](/pb-quiet-morning-0e3fea6c17b1/an-ordinary-lunch.webp)

A small serving of pasta, with the rest of the day still ahead.

In a [semaglutide trial in adults without diabetes](https://pubmed.ncbi.nlm.nih.gov/33567185/), people taking the medicine lost more weight than those given placebo, a treatment without the active drug. The adults enrolled had either obesity or excess weight alongside an associated health condition. Help with eating and physical activity was part of the trial for both groups. That’s a finding about a particular group, rather than a forecast of your result.

The [semaglutide weight-management label](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=ee06186f-2aa3-4990-a760-757579d8f77b) and [tirzepatide weight-management label](https://dailymed.nlm.nih.gov/dailymed/drugInfo.cfm?setid=487cd7e7-434c-4925-99fa-aa80b1cc776b) set out who the products are for. The adult uses include obesity, or overweight with a weight-related condition. A wish to weigh less doesn’t, on its own, establish a medical reason for treatment.

The labels also describe nausea, vomiting, diarrhea and constipation as common problems. Stomach symptoms can be severe, and fluid loss can harm the kidneys. Pancreatitis, which means inflammation of the pancreas, and gallbladder problems are among the serious warnings.

I want these details beside the benefits. They’re part of the same decision.

## Stopping is part of the story

A [trial of continuing or stopping tirzepatide](https://pubmed.ncbi.nlm.nih.gov/38078870/) enrolled people who had completed an initial treatment period. Those switched to placebo regained weight; those continuing treatment lost more. It’s a reason to discuss what happens later, without assuming that the trial decides how long any one person takes it.

Both labels carry a boxed warning, FDA’s strongest label warning, about thyroid tumors seen in rodents. Whether the medicines cause these tumors in people is unknown. The labels rule them out for people with a personal or family history of medullary thyroid cancer, or MEN 2, a rare inherited condition.

A licensed U.S. clinician can weigh the medical reason, other medicines and risks, then discuss an FDA-approved product when appropriate. Telehealth care also depends on the clinician being legally able to treat someone in their location. [Compounded versions are not FDA-approved](https://www.fda.gov/drugs/human-drug-compounding/compounding-and-fda-questions-and-answers).

I’m interested in the benefits. I also want room to say a treatment may not fit.
